What is SMILE (SMall Incision Lenticule Extraction)?

SMILE, sometimes referred to as ReLex SMILE, is a minimal invasive eye-laser-treatment to correct for refractive errors. The surgery is performed through a 2 mm micro-incision. SMILE is an innovative improvement of FEMTO LASIK. It is therefore the most advanced and gentle eye-laser-surgery for a life without glasses and contact lenses.

At the AOZ Eye-Center in Heidelberg we use the newest ZEISS VisuMax technology to perform SMILE. Our top-surgeon is very experienced with this surgical procedure. He is LASIK-TUEV SUED certified high-volume SMILE and LASIK surgeon with over 1000 procedures a year.

What is so special about the SMILE Laser?

Next to safety the AOZ Eye-Center Heidelberg offers with ReLex SMILE a maximum reduction of surgically induced side-effects:

The ZEISS VisuMax laser creates in a first step a precisely defined “Lenticle”. This is basically a small piece of tissue, which is responsible for your refractive error. Thereafter the laser creates a 2 mm micro-cut in the periphery of the cornea. Via this keyhole the surgeon extracts that piece of tissue with the use of special instruments. The laser treatment takes only 28 second and is absolutely painless, nor can you hear or feel anything.

With SMILE the creation of a flap, as with LASIK, is not necessary anymore. Therefore the surface of the eye is spared and side effects like Dry-Eye are noticeably reduced. Flap induced complications cannot occur.

* As with other Eye-Laser-Treatments you should be 18 years or older and your refraction should not have changed considerably lately. Certain chronic diseases, which can influence the eye, should be ruled out.

What happens during the SMILE Procedure?

Free consultation and evaluation

You’re invited to stop by for a free consultation and evaluation of your refractive error. During your visit we will evaluate if you are a candidate for a SMILE procedure. We will explain all aspects of the procedure and answer your questions. Please give 1 hour time.

Pre-OP exam

Prior to your surgery we do a very extensive and comprehensive exam of your eye. We take measurements of all the relevant parameters and do a thorough medical examination of your eyes, including visual acuity measurements. It is important that you cease using soft contact lens 1 week prior to the exam. You should not wear hard contact lenses anymore 2 weeks prior to the exam.

Smile surgery

We prepare your eyes and eyelids with anesthetic eyedrops, so you can’t feel any pain. If desired you can also have some medication to calm you down. After that we positioned you on the bed of the ZEISS VisuMax Laser. Starting with the right eye a small speculum will be inserted. You will be asked to follow a green blinking light and a soft interface will be applied between your eye surface and the laser system. The creation of the Lenticle takes 28 second and it is completely painless. After positioning the eye under the surgery microscope the surgeon extracts the Lenticle. All together the treatment takes 3 to 4 minutes per eye. After surgery you already have a better vision than without glasses, however for the remaining day the image will be blurry. A bandage is not necessary.
After surgery you can have a seat in our waiting area, relax,  have a drink or leave the clinic.

What happens after the SMILE treatment?

4 – 5 hours after surgery you might experience some mild irritation of the eyes,  which ceases spontaneously. You apply the prescribed eyedrops into both eyes as a recommended. At day one after SMILE surgery you already experienced a pretty good visual acuity, which will improve further within the next 5 to 6 days. We followed you up after surgery at day one, after one week and after one month.
